Nutrition services provided by LM Health and Life, LLC include the following:
assessing the nutritional needs of an individual or group,
establishing and prioritizing goals that meet the nutritional needs identified,
providing counseling in health and disease,
developing, implementing, and managing nutrition care systems, or
evaluating and recommending changes for establishing high-quality nutritional standards and services.
LM Health & Life, LLC will limit the sharing of information to that which is necessary to provide nutrition counseling for referral conditions or clients’ health goals and is within the scope of practice stated above. Sharing of information is limited to clinicians and technicians who manage clients' care or for health insurance billing purposes. A release of information must be completed for information to be shared with family, friends, or other individuals not previously mentioned.
A release of information or other form of written authorization must include a recipient, what information can be shared, how that information is to be shared (verbally, written, notes, tests, etc.), and when the information being disclosed can be released (a specific date or date range). The minimum amount necessary does not apply to information shared with healthcare providers for treatment or the client. The only exception to family and friends is an individual who has medical power of attorney over the client; evidence of that must be provided before any information can be disclosed.
LM Health & Life, LLC is committed to protecting clients' personal health information and personal identifiable information. In doing so, health information will be stored on HIPAA compliant software. We ask that clients not share information on platforms they are unaware of being HIPAA compliant (e.g., personal email or social media).
